Forms

First of all, it is useful to know what Forms are and do. A Form is a questionnaire. We have various question fields so that you can create extensive questionnaires for your coachees, customers and clients. Below are the options.

  • Section (allows you to share information without asking a question with a field)

  • Single line text field (for short answers)

  • Multi-line text field (for long answers)

  • Multiple answer (check multiple answers)

  • Single answer (select one answer)

  • Dropdown (choose an answer from a dropdown menu)

  • Yes/no (self-explanatory)

  • Ranking (provide options or let your coachee enter answers and rank them)

  • Scale (from x-x, you can enter the numbers yourself)

  • Assessment (let your coachee assess, choose symbols yourself)

  • Contact details, such as email, telephone, website, number and date)

  • Legal (let your coachee agree or not)

  • Media upload (have your coachee submit an image or document)

Using Forms

  • Open the Track

  • Add a Form as an attachment in a message

  • Create a one-time Form for your Trackee

  • Send the Form

  • Have the Form completed by your Trackee

Disadvantage: you have to make the same form again and again. Solution? Use templates.

Form Templates

Create a Form Template once and place it in Tracks unlimitedly. This is extremely useful for things that you want to use more often. Think of standard questionnaires, general terms and conditions, coaching agreements, intake forms, reflection forms and so on.

Once made, can be reused again and again.
